Fall Letter to Parents
Dear Parents:
If your child wishes to participate in Middle School or High School interscholastic athletics for the 2024-2025 school year, New York State Education Department regulations require your child must have a completed School Physical Form (examination completed within one year of sport start date) and completed Interim Sports Form (submitted no earlier than one month prior to sport start date) for the designated sport. The School Nurse must complete and receive the School Physical Form and Interim Sports Form before a student can begin to practice.
Students may take the School Physical Form to their private Doctor and then submit it to the School Nurse. School Doctors review all submitted physicals. Students in need of a current School Physical from our school Doctor must submit a completed and parent signed Interim Sports Form and a parent signed Permission for School Physical Form on the following dates to be seen by the School Doctor. All forms can be sent via email or delivered directly to the school nurse.
